The Family Assistance and Parent Support Program
Documents to the Court
The Family Assistance and Parent Support Program will submit to the courts the following documents as a complete package. Documents will be standardized and in most cases, packages submitted to the courts will consist of similar materials, which will be previewed and approved by the professionals involved with the program prior to presentation to the court. The review and pre-authorization of the documents will make it easier and faster for the court to process, as the court will already be familiar with the documents being presented.
The following is a list of the documents that will be provided to the court:
1) Parent Responsibility Plan reviewed and endorsed by parenting professionals. (Includes terms & conditions, understandings, schedule, support arrangements, etc.)
2) Consent to participate in program signed by at least one of the parents.
3) Consent to act as Family Assistance and Parent Support Agency signed by duly authorized representative of the agency.
4) Consent to act as program management team signed by two family-care professionals.
5) Consent to act as parent coordinating team, signed by the Family Coordinators who have been assigned to assist the family.
6) Request for the services of the Family Assistance and Parent Support Program by at least one other relative or step parent of the (child/children). This form may be waived if there are no relatives living in province where the child resides.
7) Consent to provide independent legal advice signed by a legal representative of each party consenting to program.
8) Any other optional forms or documents that may be applicable for the circumstances signed by those authorized (e.g. daycare consent, etc.)
